Employee Rate Authorization Form
This Member/employee will be staying at your hotel arriving (date) and departing
(date) .
(To be signed by the manager or supervisor of the person booking the rate unless the traveler is the owner of the hotel. In such an
instance, the owner may sign their own authorization.)
By signing this form I confirm that my hotel participates in the Employee Rate Program or that this
person is a Best Western corporate employee and has read the rules of this program.

Employee Obligation: Employees, and guests of employees, shall demonstrate the highest level of respect for
property and staff. Failure to comply with the Employee Rate Benefit Program Rules shall
constitute a violation of work rules and be subject to applicable penalties including, but not
limited to, revocation of travel privileges under this program.
Hotel Instructions: Retain this form in the case it becomes necessary to report program abuse.
In the event of program abuse, send an email to employeetravel@bestwestern.com. Be sure to
include:
• Your property number, your name and title
• The employee’s confirmation number and stay date(s)
• The property name and number of the employee
• The name, email address, phone number, and title of the employee
• A description of the alleged abuse
Program Rules:
• Advance reservations required. Bookable for Members only through the MemberWeb “Reservations/Book Employee Rate” Menu
item. Corporate employees must book through insidebw.com under “Employee Discounts.”
• Maximum of 1 room per stay is eligible for the EP rate. Employee must be occupying room.
• Reservations may be for no more than 5 nights in any 30-day period at the Employee Rate at the same hotel, and no more than
7 days total at the same hotel during a rolling 12-month period.
• Employees of participating hotels & corporate office employees can make reservations only for hotels that are 30 or more miles
from their place of employment.
• Rates do not include tax, and the employee/Member must pay for all taxes and incidentals in addition to the employee rate.
• The employee rate cannot be used for business travel.
• This authorization form and Photo ID are required at check-in.
• Failure to comply with all Employee Rate Benefit Program Rules constitutes a violation of work rules and, potentially,
fraud.
