VOLUNTEER RELEASE AND WAIVER

Humanity’s Team will sometimes take photos, video, or recording of activities. By

signing this form, you give us permission to use photo and video footage of you at
our sole discretion. We also have all right, title and interest in any and all
photographic images and video or audio recordings.
Volunteer, for him/herself and his or her legal representatives, spouse, heirs and
assigns, does hereby release and forever discharge and hold harmless Humanity’s
Team and its officers, directors, trustees, employees, agents, insurers and
representatives, successors and assigns from any and all liability claims and
demands of whatever kind of nature, either in law or in equity, which arise or may
hereafter arise from Humanity’s Team.
Volunteer understands that this Release discharges Humanity’s Team from any
liability or claim that the Volunteer may have against Humanity’s Team with respect
to any bodily injury, personal injury, illness, death, or property damage that may
result from Volunteer’s activities with Humanity’s Team, whether caused by the
negligence of Humanity’s Team or its officers, directors, employees, or agents or
otherwise. Volunteer covenants not to bring any action against Humanity’s Team for
any such injury or damage. Volunteer also understands that Humanity’s Team does
not assume any responsibility for or obligation to provide financial or other
assistance including but not limited to medical, health or disability insurance in the
event of injury or illness.

Diversity, and the Oneness Declaration. I am in alignment with these core
principles and strive to embody, to the best of my ability, the fundamental truth of
Oneness in my everyday life and in my work with Humanity's Team. Additionally, I
agree to the Volunteer Release and Waiver.**
