Subject : Hazards from inspection/routine maintenance of lifeboats and fast rescue crafts
Dear Captains,
This bulletin is to share the learnings from Industry incidents published by Total Energies regarding two
incidents caused by broken lifeboat slings.
Incident 1
The lifeboat, while being lifted, fell due to the breakage of four lifting slings of the lifeboat, causing a fatality.
According to the incident investigation, the steel wires were wrapped with plastic sheathing, and therefore no
sign of corrosion could be found prior to the incident.
Incident 2
When carrying out a free-fall lifeboat drill, the wire rope slings holding the lifeboat failed and it fell
approximately 14 meters to the water, causing serious injury of two crew members. The reason for failure of
the steel wire rope was the weakened ferrules near the eye splice.
Detailed incident report with corrective and preventive actions and the standard good practices are
explained in the attached reports.
Please make sure that the steel wires on board are not wrapped with plastic sheathing.
We request you to verify the condition of the slings on board near the ferrules.
