PARENT CONSENT FORM

(To be filled in by the parent/guardian and a scanned copy mailed to

the school principal@sgconline.ac.in by the 25-08-2021.)

Class ……………….. Section …………….., hereby give my written consent for
my ward to re-join the boarding section at St. George’s College, Mussoorie
on 7th September, 2021 to continue his schooling. This decision has been
solely mine and there has been no coercion or compulsion from the school. I
am confident that the school will implement the SOP set out by the
Uttarakhand Government in this regard and adhere to the instructions
contained therein. I am aware that in spite of all the precautions undertaken
by the school there may be a possibility of my ward getting infected. In
this regard I authorize the school to arrange all medical emergency
procedures. I will not hold the school authorities responsible for such
unforeseen incidents. Attached with this consent form is my ward’s medical
report. I will ensure that my ward undertakes the COVID 19 test 48 hours
before he enters the school and that a negative COVID19 test report is
furnished to the school office before he is permitted entry into the
school premises on the 7th September, 2021.
