course hereby solemnly affirm and declare that:

1. My admission is provisional as there is non physical verification of certificates and other relevant documents
due to COVID-19 pandemic lockdown period.
2. All information furnished by me in the Admission Application Form submitted on the P.K. Roy Memorial
College Intermediate Admission Portal is correct, complete and properly verified by me.
3. All certificates/documents required for my claim in the Application Form are under my possession at the time
of applying for admission to the course.
4. I will produce the required certificates/documents in original copies whenever asked by the College.
5. If I fail to produce any certificate/document in the original copy, whenever asked for verification my provisional
admission will be immediately cancelled by the College.
6. No extra time will be given to me to produce the required documents and no further communication will be
entertained in this regard.
7. If any document/s produce by me or any claim made by me for the purpose of admission in P.K. Roy
Memorial College in intermediate XIth Class and found false, forged or wrong, I will be liable for legal action
by the College.
8. In case of my provisional admission is cancelled due to non production of original certificates/ documents
required by the College authority for the admission the admission fee deposited by me will be forfeited and I
will not make any claim and will be not allowed to reapply for the course in future.
9. I have read all the terms and condition stated above properly, and, I have understood all the matters stated in
above para and agree to abide with them.
