ABSTRACT

The Mislang clinic Application is a web- based A digital keeping record service

system where the doctor and medical assistant can easily find the previous record of the

patient, while the patients can potentially increase to avoid long queues and prevent

infection in others sickness people to their check-up, in this web with a medical

assistance have more convenient way to find files of the patient. This web application is

an alternative solution for the existing issues that arises in the traditional manual

medical system and because of looking for paper, this is also one of the ways in which

checking up takes time process such as the medical assistant difficulty in finding a

papers critical hours and downtime caused by the due to finding old papers or if they

have been lost and thrown away around, while finding for a patient it takes a long time.

The proponents used the Rapid Action Model as the guide in the development process

of the research and the planned steps are Planning, where the proponents conducted

research and analysis for the current system in the transportation industry. Design and

Coding in which the proponents initialized the design model for the system and

conducted the actual coding and programming. The last step is the Implementation,

where the proponents conducted a simulation process in which the system is tested and

utilized to confirm and assure the functionality along with the survey evaluation which

resulted to an over-all average of 3.88 with the verbal interpretation of Strongly Agree.

CHAPTER 1

INTRODUCTION

Nowadays many systems have been developed to make life easier. Some

medical clinic still do not adapt the high technology. Particularly in most medical clinic

facilities, record of patient are still done on paper. We all know that modern clinics are

now operating at great pace striving to serve as many patients as possible with the best

of their abilities. But as the years rolled by, the number of patients has grown and

various medical cases arise that the manual method of managing patients’ records,

prescriptions and appointment schedule, is no longer practical.

In this study, we hope to develop a web-based system that will minimize all

paper works and manual records keeping, therefore allowing doctors and medical

assistant ease inkeeping track of patients, reducing patients’ waiting time and increasing

the number of patients served – a system that is fully automated, user-friendly, time

effective and efficient.

1.1 BACKGROUND OF THE STUDY

Mislang Clinic Management System is a community based and patient directed

organization. It is usually the first point of contact between residents of the community

and other health care facility levels. The implementation of the Mislang Medical Clinic

Management System provides a quicker and more systematic way to keeping record

the medical information of the patients. It also provides reliable and secured personal

medical records. The system will include the Patients medical records such as the name

of the patients, gender, address, date of check-up, contact number, illness patients

have their own copy of printed prescription. This information will be stored on the

centralized database server so every time the authorize person will do checking the

personal medical records information of the patients; it will be easy for them to do it.

Mislang Medical Center are using manual procedure to get information from their

patient.

The researcher, developed this system in order to make the Mislang Medical

Clinic acquire modern technology, to consume the time works and easily check monthly

reports and records. One reason why researcher did this is because patients' records

may be lost because they are only recorded in the log book. The provision of high

quality health care services is an increasingly difficult challenge and outcomes of health

care services

1.2 STATEMENT OF THE PROBLEM

What are the problems encountered by the people in the Health Center in

terms of the following?

 Medical History Records of Patients records are missing and often gives a lot of

consuming time to find it.

 Manual recording of patient’s data can be tedious and in efficient.

1.2.1 General Problem

The general problem with this study is that the records of patients is possible to

lose and the records of patients is difficult to find. The study aims to create Mislang

Medical Clinic: A Digital Keeping Records Information System that will serve as a

proposal to help the Medical Assistant and Doctor save time to find the record of

patient.

1.2.2 Specific Problems

Specifically, the study aims to:

 To be able to record a patients medical history and information in the system

 To be able to allow the Medical Assistant and doctor to add, delete and edit the

complete patient history instantly.

 To be able to add and edit the laboratory Test. Print the Prescription.

 To be able to easily find the record of patient.

 To be able to provide a chart that will serves as a list of patients’ reason of

illness per month.

 To be able to provide security regarding to patients’ data and records.

1.3 OBJECTIVES OF THE STUDY

The objectives of the study is to develop a system that would help patients and who

work in Mislang Medical Clinic. To get out from manual process and implement a new

way of method to the MISLANG Medical Clinic System The study aims to develop a

system that allows to:

 Easily to get the information you want to find.

 Lessen the time to find the records in log book or in their paper works of

patients.

 The records of patients will be organize.

1.3.1 Specific Objectives

The following statements are the specific objectives of the study:

 To create and design a system that will allow the medical assistant to

add/delete/edit of patient.

 To create and design a system that would help medical assistant and doctor

easily find the record of patients.

 To create and design a system that would help the patients keep their records.

1.4 SCOPE OF THE STUDY

The proposed system “MISLANG CLINIC MANAGEMENT SYSTEM” is a data

processing system that helped the establishment in automating its information and

transaction and created a useful and effective system that ensure the convenience of

the establishment.

 The system focused on the information tasks only.

 The medical assistant can add/delete/edit the patients information and can

add/delete/edit the laboratory test.

 The doctor can print the prescriptions of patient.

 The medical assistant and doctor can only access the system with the

supervision of admin.

1.5 LIMITATION OF THE STUDY

 The system focused on the information tasks only.

 Buy medicine is not included in the Digital System.

 SMS, Phone Call and Reservation are not included.

1.6 SIGNIFICANCE OF THE STUDY

The outcome of the study will be definition of the Mislang Medical Clinic: A

Digital Keeping Records Information System in Cabuyao, Laguna. The findings of this

study will be great help of the following:

For the medical assistant and doctors, this is an alternative system that will help

to find quickly and determine what record a patient's previous illness. It is also one of

the things that helps to be safe to not lose the important documents of the sick

patients. It helps to speed up their work because it helps them to record the illness of

person and to know their previous illness. Because of our system, it will be possible to

find the previous record of the patient more quickly or find out if they have checked up

before at the Mislang Medical Clinic. The medical assistant and doctor can use this

application if the patient has just had a check-up and she/he wants to find her record.

For the patients, this is an alternative system to keep their record and to avoid

losing their records. It helps to find their records easily if they want to get their medical

records.

For current researchers will use this research as a guideline and a reference to

develop their research literary skills.

For future researchers, this system can serve as their guide, reference, and

source of information in making similar designs of systems in a study and projects. It can

also help to widen their ideas and put into practice what they have learned

theoretically.

For our country, this research will serve as a steppingstone to achieve further

milestone in creating and designing a web-based system that will benefit not only the

users involved but also the country’s state of economy.

1.7 DEFINITION OF TERMS

1.7.1 Operational Terms

Decentralized - A practice model in which a pharmacist located on the ward is

responsible for both clinical and distribution services, including order validation and

possibly order entry.

Patient data - Is medical information held about an individual patient.

Immunization record - Means any record regardless of source documenting the status of

individual persons.

Medical records - Are the document that explains all detail about the patient's history,

clinical findings, diagnostic test results.

Patient monitoring - System is a set of systems and/or processes that enable medical

professionals to monitor a patient's health.

Clinical documentation (CD) - Is a record detailing a medical treatment, medical trial, or

clinical test.

Rural area - Is an open swath of land that has few homes or other buildings, and not

very many people.

Health care - The maintaining and restoration of health by the treatment and prevention

of disease especially by trained and licensed professionals (as in medicine, dentistry,

clinical psychology, and public health)

1.7.2 Technical Terms

RAD MODEL - is a form of agile methodology that focuses on constant iterations and

prototypes based on user feedback.

DATABASE - is an organized collection of structured information, or data, typically stored

electronically in a computer system.

C-SHARP- is an object-oriented programming language from Microsoft that enables

developers to build applications that run on the. NET platform.

MYSQL - is a Relational Database Management System (RDBMS) software that provides

many things.

CLINIC SYSTEM - is essentially software that helps clinics streamline their operations and

deliver better healthcare.

CHAPTER 2

RELATED LITERATURE AND STUDIES

Related literature and studies are presented in this chapter that tackles

different ideas, concept and conclusions about A digital keeping records information

system, web-based system, web-based software, patients record and other relative

topics in this research. This literature and studies served as the guide and inspiration for

the researchers during the process developing the system.

2.1 Related Foreign Studies

As reported by Rong Holin et al. (2022), in their title Addressing challenges in the

development of health information systems in The Gambia. In their papers they says

that The Gambia, like many other developing countries, has encountered numerous

challenges in terms of developing effective and efficient health information systems.

Their system is relevant to our system because it is record-keeping methods.

According to Addo and Agyepong, (2020), Lower patient waiting time,

minimize user burden and provide faster medical records, all of which improve patient

care. Their study is to assess the effects of information and Communication Technology

on Health Service Delivery at Tafo Government Hospital.

According to Hussain Mohammad AbuDalbouh (2019), Standardized Electronic

Health Record for Kingdom of Saudi Arabia” as Electronic Health Records are electronic

versions of patients. His system will improve clinical documentation, quality, healthcare

utilization tracking, billing and coding and make health records portable. The core

components of an electronic health record include administrative functions,

computerized physician order entry, lab systems, radiology systems, pharmacy systems

and clinical documentation. The proposed electronic copy of the patient's health record

includes all of data and important information of the patient and the patients' history

and the history of the family in cooperation with the doctor. This is relevant to our

current system because our system is to keep the record of patient, have laboratory

test, quality system and to improve mislang center.

According to Isabel C. P. Marques & João J. M. Ferreira (2019), The majority of

research in digital medicine has been focused on integrated management, electronic

medical records and medical images, but a research trend is observed in new areas such

as virtual service, the use of portable devices as instruments for monitoring the patient

and concern about the privacy of medical documents. It is relevant to our study because

our system focused on medical records but our system dont have medical images

because we print them.

According to Punit Gupta, and Dinesh Kumar Saini (2022), their study is to

develop a blockchain-based system that helps in the security and management of

patient data. This system uses blockchain technology with smart contracts to establish

an iterative, secure, accessible, and decentralized healthcare ecosystem. People will be

able to freely and securely share their medical records with doctors, hospitals, research

institutions, and other groups while having complete control over their medical data

privacy. On the other hand, the study of Punit Gapta and Dinesh Kumar Saini is slightly

different to ours, for we only check-up and keep their record but we don’t have chatbot

that will talk to patients.

2.2 Related Local Literature

According to Sheily Mendoza, Ranzel Cloie Padpad, Amira Jane Vael, Cindy

Alcazar & Rolando Pula (2019), A Web-Based “InstaSked” Appointment Scheduling

System at Perpetual Help Medical Center Outpatient Department. This study designed a

new web-based appointment scheduling system the “Insta Sked” which could reduce

the waiting time experienced by patients. It is designed for patients (booking their

appointment), medical secretaries (managing patient list), doctors, and management

(monitoring patients). The system used an integration of the Six Sigma methodology,

DMADV (define, measure, analyze, design, and verify), and BPM (business process

management). The study of them are slightly relevant to our study because we don’t

have Appointment Scheduling but we are wed-based system.

In agreement with Aaron et al., 2018, in their title Pampanga’s Barangay Health

Information System. In their system I see that we have similarities like they have patient

information, visit date and diagnosis. But their system is Barangay Health Center so their

system have chart of Gender , People in Barangay and Age Groups (Number of Cases)

and we don’t have features like this.

According to Mark Roosevelt Q. Garcia, Kurt Laurence D. Jimenez, Rein Ryan O.

Simacon, Miguel Carlo C. Vito and Emeliza R. Yabut (2021), B-Health - A Framework for

Mobile and Web Application for Barangay Health Center in the Municipality of Laguna.

The researcher proposes a study that will develop a mobile and web application for

Barangay health centers in rural areas. The features of system is to update them, patient

monitoring, deliver health announcement, monitoring patients mdical history and

conditions, can receive health announcements using short message.

According to Alma Christie C. Reyna (2023), Assessment of Online Maternal and

Newborn Immunization Schedule Records Management System. This study aimed to

assess the effectiveness of an Online Maternal and Newborn Immunization Schedule

Records Management System in a healthcare setting. The system was designed to

improve the accuracy and completeness of immunization records, enhance the tracking

and patients. It is relevant to our system but the difference is their system is about

newborn and our system is medical clinic.

Conforming to Soriano et. Al, (2018), in their title Barangay Health Management

System. The features of their system are keeping records, manages information and can

send documents. It is an online and offline system.

RESEARCH METHODOLOGY

3.1 Software Methodology Used

This chapter represents the project design, project development, and the

procedure for operation, testing and evaluation states that Rapid Application

Development (RAD) is an effective methodology to provide much quicker development

and higher-quality results than those achieved with the other software development

methodologies. It is designed in such a way that, it easily takes the maximum

advantages of the software development. The main objective of this methodology is to

accelerate the entire software development process. The goal is easily achievable

because it allows active user participation in the development process. The Rapid

Application Development Model is broken down into different phase. Basic phases of

Rapid Application Development are as follows: Requirement Planning, (user design and

construction), execution.

3.1.1 The Process Steps

Requirements Planning

During this initial stage, after we selected our team members. The Researchers

assigned tasks and assignments to each other. After the researchers did their

brainstorming about the title of the system, research and informal interview to

determine the current situation and problems encountered in Mislang Medical Clinic

located in Mabuhay Phase 1 Baclaran, Cabuyao Laguna.

User Design

In this phase, the user interface layout of the application for the doctor,

Medical Assistant and medical clinic is designed. The proponents studied every detail

and created a simple yet comprehensible layout so that the respondents will be able to

utilize the application conveniently.

Construction

In this phase, the actual program coding is conducted, and the functions of the

application are evaluated, from the functions for the Medical Assistant user interface to

the doctor’s user interface, the two main user of the system are separated accordingly

and the proponents provided different functions based on the needs and requirements

of each user.

This is the last phase where the application is finished and ready for

deployment. Prior to implementation, the system will undergo initial and final testing to

check if the functions are working accordingly. The last phase that the proponents

conducted was the completion of system construction because implementation stage

will only begin upon endorsement of the said system to a possible client.

can also update the patient profile information.

doctor account can access the system and create an account, login and log out the

doctor account can also access the main function where doctor can access the Patients

Information laboratory test, medicine of the patient and added information and also

the prescription of the patient can print instant on the system.

3.3 Software Development

The proponents used C# and SQL Server in designing and development of the

system. C# is an open-source software tool and C# is a database management tool for

MySQL compatible databases.

3.4 Project development

The Following step-by-step procedures were done during the fabrication of the web-

based system.

3.4.1 Hardware Development

1. Created a Web-Based System for Mislang Medical Clinic.

the application

In figure 3.10, the Hierarchy Diagram of the System Application is illustrated.

Starting from logging-in, the doctor can view information of patient. Starting from

logging-in, the medical assistant can view information and view prescription.

3.8 Gantt Chart for Implementation Plan

Appendix A contains Gantt Chart that presents the implementation plan of the

system. The plan covers a period of 11 weeks. In the first three weeks, communication

plans, interviews, research, and brainstorming were conducted. The planning plan was

then conducted on the following second to fifth week, including observation, project

charter, feasibility study, and final project plan. The modeling plan was then conducted

in the sixth weeks, where the data flow diagram and requirements were determined. In

the seventh and eight weeks were spent on the data gathering, conclusion and

recommendation. In ninth weeks were spent on the system we check if there’s an error

again) and check the manuscript, while the remaining weeks were spent on the

construction and deployment plan, during which the system was fixed and tested and

the study was completed.

3.9 Operation Procedure

Listed below are the step by step procedure in operating the web-based application

system for Mislang Medical Clinic:

1. Open the web-based system and choose between Medical Assistant, Doctor and

Admin,

2. Enter the Medical Assistant.

3. Enter a valid user name and password.

4. The home for Medical Assistant will appear where the Medical Assistant account has

access to the following functions:

3.10 Testing Procedure

The proponents conducted testing procedure to ensure the performance of the

Web-Based System for Mislang Medical Clinic. Alpha test or initial testing and Beta test

which is the final testing is conducted correspondingly to make sure that the application

is working as intended by the proponents.

Alpha testing is where the web-based system is checked and fixed the system

bugs and errors. The proponents also checked if some input errors or records were

safely stored and if it works properly. The proponents tested the system if there were

some errors or bugs that might occur on its program structure. All the buttons in the

web-based system is also tested if it is responding and functioning properly. The real-

time data is also checked if it reflects in the system, which is the online database of the

web-based system.

Beta Test

This is the final test in which the actual testing for the Doctor and Patient

account of the developed web application is conducted. An actual simulation is

conducted by the proponents to confirm that the functions are responding accordingly.

For the patient account, the proponents tested the functions by creating an account,

editing the user patient profile and the laboratory and prescription was save and check

if the buttons are functioning properly and to confirm that the data are being saved into

the real-time database. While for the doctor account, the proponents confirmed of the

documents of the patient in the previous process is reflected in to the patient account.

3.11 Evaluation Procedure

The proponents conducted survey to determine and evaluate the web

application’s functionality, reliability, maintainability, usability, and security functions.

With the use of the survey method, every function and content of the web-based

system is analyzed to determine whether it is still in need of improvement or properly

functioning as intended by the proponents.

In the Table 4.1, the proponents utilized the 4-point Likert Scale System to

evaluate the web-based software.

RESULTS AND DISCUSSION

This chapter presents the detailed content description and structure of the

software and as well as the capabilities of the system, the evaluation of the system

software is also presented with the use of the post-survey evaluation result to

determine the functionality, reliability, maintainability, usability, and security of the

Web-Based Software.

The web-based software of the proponents named Mislang Medical

Clinic: A Digital Keeping Records Information System, and its main function is to help

patients who check-up to clinic and as well as to doctors and medical assistant find an

innovative way of keeping records of patient and find it in the system whenever they

need it.

Assistant can add medical assistant because there are some OJT’s. Medical Assistant

4.2 Software Capabilities

The web application software is designed to give the patients and doctor

a convenient way in keeping records specifically for the patients. With the use of the

technology, the medical assistant can easily access a patient’s information just by

opening the web application. The doctor and medical assistant can easily add the

patient information with keeping record. With the help of the application software, the

whole transaction is much faster compared to the traditional way which is not

convenient and risky.

4.3 Software Evaluation

The chosen categories of respondents are the medical assistant, total number of

respondents are fifty (16) with five (5) male and ten (10) females.

proponents to be able to determine the average degree of opinion of the

respondents.

The proponents developed a Digital Keeping Records Information System for

patient information services to improve the current situation of the patient system,

help patient to conveniently keeping record service and to help medical assistant doctor

to increase their income, with the help of the technology. Doctors and Medical

Assistant will save time when finding the records of patients. The doctors and Medical

Assistant will no longer using a manual. In the development process of this research,

the proponents used the Conceptual Framework Model (See Figure 2.1) as the guide in

the step-by-step planning and execution for the whole research, the proponents also

created a Gantt Chart for an effective Time Management (See Appendix A), and as

planned, the research began last February 2023 and planned to finish it on or before

April 2023. The processing was executed smoothly, and the research was concluded in

the target completion date. For the construction of the system, the proponents used

the Rapid Action Model (See Figure 3.1) and the planned processing steps are Planning,

Design & Coding and Implementation. The project’s Feasibility and Benefit was also

analyzed, and the benefits that will occur in the future.

Given the Summary of Findings the proponents conclude that the web

application system medical information system of keeping record service has a potential

to improve the proponents developed a web-based application for patient information

services to improve the current situation of the manual of keeping patient record, help

patient to conveniently keeping record service and to help medical assistant doctor to

increase their income, with the help of the technology. In the development process of

this research, considering that these manual procedures of keeping record are very

common in most areas in the country. The web application for medical records service

can also solve some of the issues that both patient and doctor they encounter check-up

in a daily routine, with the help of the application, doctors can easily see the availability

of patient information. The web application software is designed to give the patients

and doctor a convenient way in keeping records specifically for the patients. With the

use of the technology, the medical assistant can easily access a patient information by

just opening the web application the doctor and medical assistant can easily add the

patient information with keeping record. With the help of the application software, the

whole transaction is much faster compared to the traditional way which is not

convenient and risky.
