THIRD RENEWAL CONTRACT OF LEASE

KNOW ALL MEN BY THESE PRESENTS:

This THIRD RENEWAL CONTRACT OF LEASE is made and executed at the City of _____, this
_______________, by and between:

_____________, represented by its General Manager, ______________, of legal age, ____________, and
with office address at _________________, hereina+er referred to as the LESSOR.

-AND-

________________________, with residence at ___________________, hereina+er collec-vely referred


to as the LESSEE.

WITNESSETH; That

WHEREAS, the LESSOR is the owner of THE LEASED PREMISES, a unit in a residen-al building, par-cularly
designated as ____________________________;

WHEREAS, the LESSOR and LESSEE entered a Contract of Lease on the _______________ for the period of
one year commencing from ___________________ hereunto a1ached as Annex “A” and made integral
part of this THIRD RENEWAL CONTRACT OF LEASE over THE LEASED PREMISES;

WHEREAS, the LESSOR and LESSEE entered a FIRST RENEWAL CONTRACT OF LEASE on the
_______________ for the period of one year commencing from ____________________ hereunto
a1ached as Annex “B” and made integral part of this THIRD RENEWAL CONTRACT OF LEASE over THE
LEASED PREMISES;

WHEREAS, the LESSOR and LESSEE entered a SECOND RENEWAL CONTRACT OF LEASE on
________________ for the period of three years commencing from __________________________
hereunto a1ached as Annex “C” and made integral part of this THIRD RENEWAL CONTRACT OF LEASE over
THE LEASED PREMISES;

WHEREAS, the LESSEE offers to RENEW the said SECOND RENEWAL CONTRACT OF LEASE and the LESSOR
accepts the offer under the same terms and condi-ons of the SECOND RENEWAL CONTRACT OF LEASE.

NOW THEREFORE, for and in considera-on of the foregoing premises, the PARTIES hereby agree as follows:

1. TERMS OF THE LEASE: That the lease period is for Three (3) years commencing from the
_________________________;

2. AMOUNT OF RENT: That the rental of the leased residen-al premises is in the amount of
___________________________ per month.

3. DEPOSIT: That the LESSEE has already paid an amount of _________________ as RENTAL DEPOSIT
which is equivalent to (1) month rental; And this deposit cannot be used as rental during the lease
period, and in case of Termina-on of the lease agreement, this will be returned to the LESSEE;

4. IMPROVEMENTS: That the LESSEE shall not make or cause to be made any improvements thereon
without the prior consent of the LESSOR, such improvements automa-cally form part of THE LEASED
PREMISES and at the sole expense of the LESSEE;
5. REPAIRS: The LEASED PREMISES is on an “AS IS WHERE IS BASIS” WITHOUT ANY EXPENSES TO BE
INCURRED BY THE OWNER NOR LIABILITIES TO BE CHARGED TO THE OWNER FOR ANY REASON
WHATSOEVER;

6. SUBLEASE: That the LESSEE is expressly prohibited to directly or indirectly sublease, assign, or
transfer his/her right under the LEASE AGREEMENT to a third party or par-es nor to anyone, without
the express wri1en consent from the LESSOR and is allowed for Two (2) persons only to occupy the
unit;

7. BILLS: That all charges of water and electricity or other services and u-li-es used in the premises
shall be for the sole account of the LESSEE; That the LESSEE shall pay the bills promptly each month;

8. PROHIBITIONS: That the LESSEE is prohibited to store flammable materials in the LEASED PREMISES
which will increase the insurance rate of the building. Also, NO PET IS ALLOWED in the unit;

a. The LESSEE shall make sure the corridors, pa-os of the building are only used as passageway or
aisle for lawful ingress and egress into and from the leased premises and the LESSEE SHALL FREE
THE PREMISES FROM ANNOYING NOISE AND SOUNDS, noxious odors and other nuisances that
will disturb the peace and tranquility of the other tenants of the building;

b. The LESSEE is only allowed to the delivery of furniture’s equipment and other bulky items only
during office hours and reserves the right to limit the weight of any equipment or other ar-cles
which the LESSEE may place in the rented unit;

c. The LESSOR is not liable for the presence of bugs, rats, ants and termites, insects or whatever
pest in any kind or nature in THE LEASED PREMISES;

9. TERMINATION CLAUSE: If in case the LESSEE will terminate this lease prior to the expira-on of the
same, he/she shall no-fy the LESSOR in wri-ng at least THIRTY (30) DAYS prior to the intended date
of termina-on. In the event of such pre-termina-on, the LESSEE shall be en-tled to a propor-onate
refund of the paid annual deposited rent payment. However, the en-re deposit shall be forfeited in
favor of the LESSOR. The LESSEE shall surrender the peaceful possession of the premises with all the
fixtures as well as proofs of u-lity bills as fully and currently paid;

10.INSPECTION: The LESSOR can inspect the premises from -me to -me with the consent of the LESSEE
within a reasonable -me;

11.That the LESSEE shall acknowledge that THE LEASED PREMISES is in good and tenantable condi-on;
That the LESSEE shall keep THE LEASED PREMISES including its surroundings always clean, with all
garbage thrown daily, and shall comply with all the laws, ordinances, and regula-ons of the city and
the na-onal government as regards the use of THE LEASED PREMISES. Failure to comply shall be at
the risk and expense of the LESSEE;

12.That the LESSEE shall be responsible for the acts, omissions of his/her family, or any other persons
allowed access to THE LEASED PREMISES. Upon termina-on of this THIRD RENEWAL CONTRACT OF
LEASE for any reason whatsoever, the LESSEE shall immediately vacate the premises and return
possession thereof to the LESSOR so the la1er can show the same to prospec-ve tenants;

13.VIOLATION: Any viola-on of the terms provided in this THIRD RENEWAL CONTRACT OF LEASE either
by the LESSOR or LESSEE shall be sufficient ground for the termina-on of the same and shall subject
the guilty party to the payment of damages, aside from the lawsuit costs and a1orney’s fees; And
That the venue of all ac-ons arising from this THIRD RENEWAL CONTRACT OF LEASE shall be brought
to the jurisdic-on of the appropriate court of _______________.
