1. Reference: Memorandum from C,DPRMD, EPD Dated September ___,
2023 with subject Conduct of Summary Hearing Proceedings.

2. This pertains to the designation of the undersigned in the Summary

Hearing Officer in the Administration case against PSMS Familara and transmittal of
original case record consisting of ____ pages for Grave Neglect of Duty (Failure to
Attend Scheduled Court Hearing) docketed under
NCRPO______________________.

3. In this regard, the undersigned respectfully request for the ten days
extension of time starting September 28, 2023 to give ample to review/resolve the
said administrative case due to the following reasons to wit:
a. The undersigned SHO only received the said case folder on September 14,
2023; and
b. Based on the existing policy the counting of 30 days period to resolve the
administrative case starts upon receipt of the case folder by designated
SHO.

4. Further, the undersigned SHO respectfully prayed for favorable action

relative to this matter.
