Physical Assault

I, Kevin Saja, 22 years old, and residing at San Pablo, Zamboanga del Sur, do hereby solemnly
affirm and state the following:
On the afternoon February 24, 2024 at 4:00pm, I was viciously assaulted by Joshua
Quilar at Caridad , Zamboanga del sur. The assault occurred under distressing circumstances
when I was at convenience store to buy something to eat he punched me without knowing the
reason, he was drunk that time, As a result of the attack, I was sustained severe injuries,
including Wounds and Bruises. The entire or deal left me in a state of intense fear and distress.
Promptly following the assault, I sought medical attention and promptly reported the
incident to the appropriate authorities. Attached here with, are the necessary medical records and
photographic evidence documenting the extent of my injuries I provided this sworn affidavit to
formally record the heinous physical assault I endured and to pursue legal action against the
perpetrator for their reprehensible behaviour.
