AFFIDAVIT

I, RAHUL MITRA, son of Late Siddhartha Kumar Mitra, aged

about 59 years, by faith Hindu, residing at Udita, 1050/1, 06/1202,
P.O. Santoshpur, P.S. Survey Park, Kolkata-700075 do hereby
solemnly affirm and declare as follows :

1. That my Father Siddhartha Kumar Mitra died intestate on

29/11/2022, and his death certificate being registration no.
D/2022/307552 is issued by department of health and family
welfare Government of West Bengal.

2. That my father Siddhartha Kumar Mitra died intestated leaving

behind surviving his following legal heirs and successors :

(i) Roma Mitra Wife (86 Year old)

(ii) Rahul Mitra Son (59 Year old)

That save and except the above named persons there is no any other legal
heirs and successors of my deceased father Siddhartha Kumar Mitra.

3. That I am Indian citizen.

Verified that the contents of my affidavit are true to my knowledge and belief
and nothing has been concealed the reform.
