Respected Sir,
I, Biswarup Singh of old police line is an employee of your bank with
good conduct. Recently on 12th September 2022 I suddenly fell ill with severe
pain in throat and stomach which I did inform my concerned authority. I
managed to consult Dr Nibarananda at Tapananda Charitable dispensary where
I was suggested to refer to Tata TMH main hospital.
I fixed a medical appointment with Dr Abhijit Kumar on 22 nd sep and also
suggested to do a MRI test. On 27th sep I again fell severely ill and was
immediately rushed to Deben Mahato Medical College Hospital Purulia.
I do hereby request you to consider my case under medical ground and
grant me leave so that I can avail proper medical treatment. I request your kind
co-operation.
