ONCE YOU HAVE RECEIVED YOUR BIRTH CERTIFICATE BACK

AUTHENTICATED FROM YOUR BIRTH STATE or VITAL CHEK
YOU CAN NOW AUTHENTICATE ON A FEDERAL LEVEL.
Before you start, obtain some certified mail labels from your
local post office

YOU WILL NEED: (all originals)

• Your authenticated Birth certificate w/auth letter

(Great Seal)
• Form DS-4194 filled out entirely (add the certified
mail tracking number sticker to ds-4194)
• A money order filled out to U .S Department of State
($20.00 per document to be authenticated)
• Self-addressed stamped envelope for return of your
document(s)
• Manila envelope

1. Put all docs, money order and self-addressed envelope in

manila envelope
 2. Address the large manila envelope to:
U.S. Department of State

Office of Authentications

CA/PPT/S/TO/AUT

Sterling, VA 20166-1206

3. You need to send Certified/Registered mail

4. Save your Tracking number as this will be how you call
and check on the status of your paperwork
5. The DOS will return your paperwork once you have been
- authenticated and you will now be a National on the
state and federal level and added to the DO NOT DETAIN
list.
