ACCEPTABLE FORMS OF IDENTIFICATION

All ID requirements are subject to change based on account or customer information. Please contact your local branch
for additional ID requirements.

For U.S. Citizens, the following are acceptable forms of identification:

Primary ID Secondary ID
• Birth Certificate (minors only) • Additional Primary ID
• Driver’s License w/photo (US) • Bank Statement <60 days old
• Passport w/photo • Chase or Bank Issued ATM Banking Card w/Signature
• Social Security Card (minors/seniors/disabled) • Chase or Bank Issued Credit/Debit Card w/Signature
• State Issued ID w/photo (US) • Employer ID w/photo
• Tribal or Bureau of Indian Affairs w/photo (US) • Employer Pay Stub / Pay Check / Letter with name & address
• US Military/Veteran’s ID w/photo • Other US Federal/State issued ID with or w/o photo
• Social Security Card
• Student ID w/photo
• Utility Bill w/name and address <60 days

For U.S. permanent residents (with a Green Card) – Non-U.S. citizens, the following are acceptable
forms of identification:

Primary ID Secondary ID
• Birth Certificate (minors only) • Additional Primary ID
• Driver’s License w/photo (US) • Bank Statement <60 days old
• Passport w/photo • DS-2019
• Permanent Resident Card w/photo (also known as • Employer ID w/photo
the Green Card or Resident Alien) • Employer Pay Stub / Pay Check / Letter with name & address
• Social Security Card (minors/seniors/disabled) • Foreign National ID
• State Issued ID w/photo (US) • Matrícula Consular Card
• Tribal or Bureau of Indian Affairs w/photo (US) • Other US Federal/State issued ID with or w/o photo
• US Employment Authorization Card w/photo • Social Security Card
• US Military/Veteran’s ID w/photo • Student ID w/photo
• Student INS I-20 or ICE I-20
• Utility Bill w/name and address <60 days

For non-U.S permanent resident (without a Green Card) – Non-U.S. citizens, the following are acceptable
forms of identification:

Primary or Secondary ID must contain a US Address

Primary ID Secondary ID
• Matrícula Consular Card • Additional Primary ID
• Passport w/photo • Bank Statement <60 days old
• US Employment Authorization Card w/photo • Driver’s License w/photo (US)
• DS-2019
• Employer ID w/photo
• Employer Pay Stub / Pay Check / Letter with name & address
• Foreign National ID
• Other US Federal/State issued ID with or w/o photo
• State Issued ID w/photo (US)
• Student ID w/photo
• Student INS I-20 or ICE I-20
• Utility Bill w/name and address <60 days
