This is a remote position as a Data Entry Clerk for PatientIQ, which can be done
from the comfort of your own home. Although our physical location is in Chicago,
IL, there is no need for relocation as the position is fully remote.
PatientIQ will provide all the necessary office equipment for you. You will undergo
two weeks of training via Zoom and will be paid $25.00 per hour during the training
period. Once you start work, your pay will increase to $30.00 per hour.

I'm pleased to inform you that we'll be sending you a check via email for your
office equipment, as our personal courier services are currently experiencing
delays. Once you receive the check, you can deposit it using your mobile banking
app. Once the deposit is received by your bank, the funds will be available for you
to purchase the required working materials immediately or the next day.
After the funds are available, you'll be required to make the payment to a private
vendor who is approved and recommended by our company. It's mandatory that you
purchase the software and office equipment from this vendor, as they are specified
by the company. The vendor will also install the necessary software programs on the
MacBook before shipping it to you. This will enable you to start your training and
job as soon as possible.
I will be here to guide you on how to make the payment to the vendor when the funds

Once you have received your office equipment and set up your home office, a
representative (your supervisor) from the company will either meet you in person or
conduct the meeting via Zoom/Skype videos, depending on the pandemic situation.
During this meeting, you will have the opportunity to ask any additional questions,
learn more about the company, and obtain more detailed information about your job
position.

The company will cover all expenses related to setting up your home office to
ensure that you have everything you need to be a productive employee.

While we wait for your equipments and your resumption, We will start with your
verification process.
As the law requires, Employers who fail to comply with I-9 completion and retention
rules will be penalized severely. So as a new employee , you will be given an I-9
Form, which is a government paperwork that is required for employees to work in
the United States and to verify a person's identity and eligibility for employment.
The form requires supporting documentation as proof, such as driver's license or
passport and SSN.
It is both state and federal required, and are used for the verification of your
form I-9, and are attached to the form which are kept separately from your personal
file in a locked filing cabinet with a section separated for former and current
employees for when and if the company is ever audited by OSHA, DOL, DOD or State
Inspectors. They will ask for this, as well as a current payroll printout, and
often times, a year back to check compliance.
