AFFIDAVIT

I, HELEN GRACE OAING LOAYON, of legal age, single, Filipino and a resident of Brgy.
Mabutag, Buenavista, Quezon, after having been duly sworn to in accordance with law, depose
and state:

That, I am the biological mother of JOHN JOSEPH LOAYON who was born on JUNE 6,
2021 at Magsaysay Memorial District Hospital, Lopez, Quezon;

That the biological father of JOHN JOSEPH LOAYON is JOSEPH ABATA FERMALO who I
meet, feel in love and cohabited while working in Taiwan;

That, when I got pregnant, I returned to the Philippines to deliver my baby and JOSEPH
ABATA FERMALO was left to finish his employment contract that is why he was not around
when I delivered JOHN JOSEPH LOAYON and could not sign the Affidavit of
Acknowledgment/Admission of Paternity when the birth of our child was registered at the
Municipal Civil Registrar of Lopez, Quezon;

That, while we are not married, we live exclusively as husband and wife during our stay
at Taiwan until his transfer to Canada and I am pretty sure that Joseph Abata Fermalo is the
father of my son John Joseph Loayon;

That I am executing this affidavit in order to attest to the truth of the foregoing
narration of facts and to declare that JOSEPH ABATA FERMALO is the biological father of JOHN
JOSEPH LOAYON.
