RE: Letter of Intent Requesting for LOA

I hope this letter finds you well. I am writing to formally express my intent to request an
honorable dismissal from Pamantasan ng Lungsod ng Maynila. I am _________________ with
student number of _________________, and I am currently enrolled in BSChE.

Due to my absence without approved leave and a failed subject, my maximum residency
requirement in the university cannot be completed. I find it necessary to seek an honorable
dismissal. I am committed to fulfilling any outstanding obligations and responsibilities before
my departure to ensure a smooth transition for both myself and the institution/organization.

I would like to express my gratitude to the entire Pamantasan ng Lungsod ng Maynila

community for the support, guidance, and camaraderie I have received during my time here. I
