One survey sheet per student
Customize your worksheet by:
• Including activities that your students can relate to.
• Including local events and place names that your students will know.
• Including any additional vocabulary that your students have learned.
Set-up (5 minutes)
Give each student a survey sheet. Write Have you ever gone parasailing? on the board, along with the three answer
choices from the survey sheet.
Have one student come to the front of the class with the survey sheet. Ask the student, “Have you ever gone
parasailing?” Circle the student’s answer on the board. Say to the student, “Ask me.” After the student asks, answer
the question and point to your answer on the board.
Procedure (20 minutes)
Divide the students into pairs. Have the students write their partners’ names on the top of their survey sheets. Have
them take turns asking and answering the questions on the survey sheet. If the answer is yes, they should ask
follow-up questions to get more information.
When students have completed the survey, dictate these answers and have students write them down at the bottom
of the survey.
Mostly “a” answers—Very adventurous! You are very daring and fun!
Mostly “b” answers—A little adventurous! You try some interesting things.
Mostly “c” answers—Not very adventurous! Try something new!
Students then add their partner’s answers and report the results of the survey to their partner.
Extension (10 minutes)
Divide students into pairs with different partners. Have them make up questions of their own and repeat the
procedure.
