1. Purpose and objectives for the safety inspection:

➢ The main objectives of safety inspection are to prevent workplace injuries and
deaths.

➢ To avoid the suffering because of these events can cause for worker, Workers
family and employer.

➢ Also prevent financial hardship of these events cause for victim, their family
and employer.

➢ Then more over save company reputation, time, profits, compensation money

2. Inspection Findings – Hazards (Unsafe conditions / Unsafe acts)

Description of Unsafe Condition/Unsafe Act:

Failure for to making safety scaffolding platform. Failure for without safety gloves catch of
casting bucket wire, working at opening edges, no proper foot hold.

Possible reasons for the unsafe condition/unsafe act (Root Causes):

➢ The worker does not follow the safe workplace safe procedure.

➢ Do not make the proper safety scaffolding erection and careless of working without
hand gloves.

➢ Faling unsafe condition.

Preventive Actions:

➢ To makes proper scaffolding platform and put on proper hand glows of casting
period.

➢ To given instructions and brief the work place Safety and working on hight.

➢ To arrange the scaffolding for around side.

➢ No cover of Rebar ends.

The workers diesel chemical can put on open working place. There is no prevention
for to used chemical items to return in chemical storage.

➢ The workers used the diesel chemical.

➢ And put on open place.

➢ To give the chance fire hazard for others.

➢ Making the proper chemical storage.

➢ To briefing workers on time for what is the hazards come for open place put
on the chemical, to shift the chemical on storage area.

➢ To inform workers follow SDS every time handling chemicals.

Poor Housekeeping in the work area.

Possible reasons for the unsafe condition/unsafe act (Root Causes):

➢ Inadequate metal bin to clear the Un wanted waste Rebar regularly.

➢ No making proper barricade on materials place.

➢ Worker put the metal here.

Preventive Actions:

➢ Supervisor need to ensure the regular housekeeping in the work area

➢ Need to increase the number of metal bin twice a week until the situation
improves.

➢ To ensure make proper barricade. To inform and give the guide for morning
TBM meeting.

Description of Unsafe Condition/Unsafe Act:

Failure for Rebar end protection cap. Rebar Workers give to the chance, injuries for
others.

Possible reasons for the unsafe condition/unsafe act (Root Causes):

➢ The worker doesn’t know he cannot use the safety cap.

➢ When the fall down steel rods might be inject their body.

➢ Steel rods maybe after rain its causes to rusting.

Preventive Actions:

➢ Supervisor need to ensure the regular housekeeping in the work area.

➢ Need to increase the number of metal bin twice a week until the situation
improves.

➢ To ensure make proper barricade.

➢ To inform and give the guide for morning TBM meeting.

4. Recommendations to prevent the repeat of the similar findings:

➢ Supervisor to remind the workers to about electrical safety during

toolbox meeting.

➢ Adequate number of rubbish bins shall be provided.

➢ Supervisor remind the workers to do daily housekeeping.

➢ To arrange more life line and advise them.

➢ Use PPE for body safety.
