AFFIDAVIT
(Re: Wrong Use of TIN)

I, APOLONIA LIVIA VILLAREZ, Filipino, of legal age, and a resident of Brgy. Buli,
Taal, Batangas, Philippies, having been duly sworn in accordance with law, hereby depose
and state that:

1. I am the only daughter of deceased LILIA M. VILLAREZ.

2. During the processing of the settlement of the estate of my late mother, when
asked to fill-out the details of his Tax Identification Number (TIN), what was recorded was
the TIN he was using when he was still alive.

3. The TIN used by my mother during her lifetime was 736-274-493.

4. What was actually needed to be recorded, however, was the TIN corresponding to
her estate.

5. I am executing this affidavit to declare the truth of the foregoing facts, for the
purpose of correcting the TIN used in the settlement of the estate of Lilia M. Villarez, and
for whatever legal purpose this may serve.
