AFFIDAVIT
(Re: Removal of Notice of Adverse Claim)

I, PATRICIA YOLANDA M. HOLGADO, Filipino, of legal age, and resident of Brgy.

Poblacion, Sta. Teresita, Batangas, Philippines, having been duly sworn in accordance with
law, hereby depose and state that:

1. On January 25, 2019, together with my spouse Diosdado M. Holgado, we caused

the annotation of a Notice of Adverse Claim on the following properties:

a-. Transfer Certificate Title No.: T- 076-2016004512

b. Transfer Certificate Title No.: T- 076-2016004513
c. Transfer Certificate Title No.: T- 076-2016004514
d. Transfer Certificate Title No.: T-076-2016004515
e. Transfer Certificate Title No.: T-076-2016004516

2. The following properties are now being offered for sale to the public.

3. In order to cancel the existing encumbrances attached to the abovementioned

properties, and to be able to dispose of them without any legal impediments, I am now
causing and authorizing the cancellation of our adverse claim on the said parcels of
land.

4. I am executing this affidavit to declare the truth of the foregoing facts, for the
purpose of cancelling the annotation of Notice of Adverse Claim on the all of the
abovementioned titles and for whatever legal purpose this may serve.
