PROCEDURE FOR REGISTRATIONS BPJS KESEHATAN

❖ Document Needed Before Register


1. Legal Document from company such as Copy of NIB, Copy of Company NPWP, Copy of
Owner KTP/Kitas (and if the owner are foreigner need passport).
2. Company Data such as phone number, emails, owner number, and owner emails.
3. Employee data such as KTP, KK, Phone Number, Emails And Salaries that the employee
receive.

❖ Procedure to Registrations
1. Representative from the company come to the office or make an whatsapp
message to +6285234084285 (Kuta Utara and Badung Utara Area) say your
identity and told them you want to make a registrations for BPJS Kesehatan.
2. They will give you all the files that need to print and you need to fill it with the
identity of company and also employee identities and data.
3. After all the data filled, all the document need to send to the office of BPJS
Kesehatan in Renon.
4. They Check your registration data and company information if there something
miss or not correct or they need some data they will inform you directly but if so
crowd you need to wait and they will send you confirmations letter from email or
whatsapp.
5. After they accept your applications they will inform you to pay some amount for
registrations and you need to pay Immediately.
6. Your application already done and BPJS Kesehatan for employee can use at that
month.

❖ Time Estimations
1. Estimate as soon 1 weeks done the process and as long 1 month done the
process.
2. More fast we got the document and the process of registrations will done
immediately.

❖ Estimation of Expenditure
1. Materai 5 Pcs = 5 X IDR. 10.000 = IDR. 50.000
2. Printing & Stationary = IDR. 50.000
3. Fuel/Petrol/Gasoline = 2 X IDR. 20.000 = IDR. 40.000
TOTAL IDR. 140.000
❖ Notes
There is some data may be needed during the process and they will inform you as
soon as they got the confirmations letters.
