You will need to provide certified* copies of proof of identification to prove you are the person

to whom the superannuation entitlements belong. This may be in the form of either one proof of
ID from list A OR one proof of ID from both list B and C:

A.  A current driver’s licence


Primary photographic  A Passport (Must be a current or if it is an Australian
identification passport it may have expired in the last two years)
 Proof of age card
 National identity card

B.  Birth certificate
Primary non-photographic  Citizenship certificate
identification  Pension card
NB. If the name shown on the Birth Certificate has changed,
(if providing ID from list B, please supply a certified* copy of either:
you must also provide ID from - Marriage Certificate (required even if the parties are
list C) separated)
- Decree Nisi, Certificate of Divorce or Decree
Absolute
- Deed Poll Documentation
C.  A notice containing the claimant’s name and residential
Secondary identification address from:
- a Commonwealth, State or Territory body or the ATO,
(if providing ID from list C, received within the previous 12 months
you must also provide ID from - a local government body or utilities provider received
list B) within the previous 3 months
PLEASE NOTE:
The Person who is authorised to certify documents must sight the original and the copy ensuring
both documents are identical, and all pages have been certified as true copies. The Authorised
person must write or stamp 'certified true copy' followed by their signature, printed name,
qualification (e.g. Police officer, Lawyer, Postal Manager, etc) and date.

All documentation must be legible and in English. If the original documents are not in English, the
following must be provided:
 an English translation of the document;
 an affidavit from the translator which:
 states his or her qualifications to translate
 attaches a copy of the original document
 attaches the translated document
 states that the translation is an accurate translation of the document, and
 states that the attached copy of the document is a true copy of the document
translated.
*A Certified copy means a document that has been certified as a true copy of an original
document by one of the following persons:
1) a person who is enrolled on the roll of the Supreme Court of a State or Territory, or the
High Court of Australia, as a legal practitioner (however described)
2) a judge of a court
3) a magistrate
4) a bailiff
5) Clerk of the court
6) Commissioner for Affidavits
7) Legal practitioner
8) Sheriff or Sheriff’s officer
9) Chiropractor
10) Dentist
11) Medical practitioner
12) Nurse
13) Optometrist
14) Pharmacist
15) Physiotherapist
16) Psychologist
17) Veterinary surgeon
18) a chief executive officer of a Commonwealth court
19) a registrar or deputy registrar of a court
20) a Justice of the Peace (Justices of the Peace in your area can be found by calling the
Recorded Information Line on 1300 365 567)
21) a notary public (for the purposes of the Statutory Declaration Regulations 1993)
22) a police officer
23) an agent of the Australian Postal Corporation who is in charge of an office supplying
postal services to the public
24) a permanent employee of the Australian Postal Corporation with 2 or more years of
continuous service who is employed in an office supplying postal services to the public
25) an Australian consular officer or an Australian diplomatic officer (within the meaning of
the Consular Fees Act 1955)
26) an officer with 2 or more continuous years of service with one or more financial
institutions (for the purposes of the Statutory Declaration Regulations 1993)
27) a finance company officer with 2 or more continuous years of service with one or more
finance companies (for the purposes of the Statutory Declaration Regulations 1993)
28) an officer with, or authorised representative of, a holder of an Australian financial
services licence, having 2 or more continuous years of service with one or more licensees
29) a member of the Institute of Chartered Accountants with 2 or more years of continuous
membership
30) Permanent employee of the Commonwealth or a Commonwealth authority, or a State or
Territory or a State or Territory authority; or a local government authority; with 2 or more
years of continuous service who is not specified elsewhere in this list.

Have you changed your name or are you signing on behalf of another person?

If you have changed your name or are signing on behalf of the applicant, you will need to provide
a certified linking document. A linking document is a document that proves a relationship exists
between two (or more) names.
