Please see attached example of report and lessons learnt document that has been prepared
from a recent incident. As you will see in the example of one of our client’s incident report
formats, that the document is much more advanced than our document.

As this is the final document it should contain all supporting documents to ensure that it is
properly closed. i.e:
Training records, Driver statements, Vehicle insurance, badge, and TBT Records

At this moment in time, I do not want to have to change our format, so it is imperative that all of
the information is correctly sourced and everything inserted in the document. You will see from
the email related to incident reporting, there is issues where reports are not being sent within
the correct timescales and not being fully completed.

To confirm the steps:

1. Immediate notification
2. Initial investigation report
3. Closed / Final investigation report (including all evidence required)
4. Event Review meeting. We will now hold an event review which will produce lessons to be
learnt going forward. Attached is the link to the folder where you will see what is required to be
input. This must be completed within 7 days of any event occurring.
Incident Logs

It is very important that we are reminded that anything that is to be sent to a client must be sent
in pdf version only. If you need to confirm anything or have anything checked / reviewed before
it is sent to anyone, then please make sure to do that.
