AFFIDAVIT OF DISCREPANCY

I, , of legal age, Filipino citizen, single , and a resident of Bukidnon, after having
been sworn to in accordance with law do hereby depose and say:

1. That I am the biological son of the deceased, who was born on July 01, 1985 at
Bukidnon and died on October 24, 2023 at the same place;

2. That the fact of her birth and death were duly registered in the Local Civil
Registry of , Bukidnon. However, an erroneous entry was made in her Certificate
of Death, wherein her date of birth appeared as October 07, 1985;

3. That the correct entry should be July 01, 1985;

4. That the names who was born on October 07, 1985 and who was born on
July 01, 1985 which is the correct entry refers to one and the same person, my
mother;

5. That I am executing this affidavit to explain the discrepancy existing in my

mother’s certificate of death;

6. Furthermore, I am executing this affidavit to state the truth of the foregoing

facts and for whatever legal purposes this may serve best.
