AFFIDAVIT OF LOSS

KNOW ALL MEN BY THESE PRESENTS:

I, Jerald M. Aquino, Filipino of legal age, single, and a resident of Zone 2

Cagbunga, Gainza, Camarines Sur after being duly sworn in accordance with the law,
dispose and state:

That as a bona fide student at Naga College Foundation Inc, I was issued a
Student Identification Card (ID) No. 21-11883;

That to my surprise, I discovered last March 28, 2024 that the said Identification
Card could not be found.

Thus, I execute this affidavit to establish its loss and to support my application
for the issuance of a new Identification Card.
