AFFIDAVIT

I, CHEPKORIR JOAN ID NO 33257772 of BOX 13-SIONGIROI ,BOMET COUNTY do hereby make

oath and state as follows:-
1. THAT: I am an adult female of sound mind and hence competent to swear this affidavit.
2. THAT: I am the holder of Identity card No. 33257772 bearing the names CHEPKORIR JOAN.
3. THAT : The names which appears in National Identity card CHEPKORIR JOAN and the

names which appears in my academic certificates JOAN CHEPKORIR are all mine.
4. THAT: I swear this affidavit in verification of my Identity that names CHEPKORIR JOAN as it
appears in my National ID and the names JOAN CHEPKORIR as in my certificates e.g. Birth
Certificate, leaving certificate, and certificates of Egerton University are all my names and also for
official use.
5. THAT: what is deponed to herein is true to the best of my knowledge information and belief.
