PARENT CONSENT AND WAIVER FORM

As a parent/guardian of the learner-beneficiary, I hereby give my consent and acknowledge

by my signature that my son/daughter of Grade 12 TVL – Prosser, _____________________
will join the Computer Systems Servicing (CSS) Training to be held at Softnet ITC, Mariveles,
Bataan starting April 3, 2024. Monday to Friday. 2pm to 5pm.

I am aware of the usual risks and dangers inherent in the said training thus, accidents can
occur with or without any fault on either he part of the students or the teachers where the
training is taking place. By allowing my child to participate in this workshop, I accept the risk
of any untoward incident and conform to the school rules and regulations that this training
is suitable for my son or daughter to enhance his/her skills as a student. I have read and
understood the terms of this consent and waiver, and understood well that is binding upon
me, my heirs, executors and administrators.
