incumbents of skilled and functionally related positions
under permanent status with CS Prof, Sub-Prof, relevant
RA 1080 eligibility

Education : Completion of 2-years studies in college or High school

graduate with relevant vocation/trade course

Training : None Required

4. Performance rating in the last rating period with VERY SATISFACTORY rating (if applicable);
5. Certified TRUE COPY of the Autheticated Certificate of Registration/Eligibility/Rating/License;
6. Certified TRUE COPY of the Authenticated of Form 137, Transcript of Records and Diploma; and
7. Copies of Certificate of Training programs attended.
