AFFIDAVIT OF LOSS

I, EMMANUEL KUIZON DANAG, of legal age, married, Filipino, and

with residence and postal address at Brgy. Mandahilag, Talisayan, Misamis
Oriental, Philippines, after having been sworn to in accordance with law
hereby depose and state:

1. That sometime this month in July of 2023, my Globe Sim Card bearing
number 09165116045 was misplaced;

2. That I used the foregoing Globe Sim Card bearing number 09165116045 for
my Gcash account, which has a balance of Php 3, 400.00, more or less at the
time the said Globe Sim Card was misplaced;

3. That I exerted all efforts to retrieve and locate said Globe Sim Card but all
efforts proved futile. Hence, such Globe Sim Card is now beyond recovery and
considered lost for all legal intents;

4. That I hereby execute this Affidavit for all legal intents and purposes this
document may serve best and I hereby attest to the veracity and truthfulness of
the foregoing facts with full understanding of its consequences.
