JOINT AFFIDAVIT OF DISCREPANCY

BY DISINTERESTED PERSON

WE, MODELINA A. LAMBINICIO and ROSARIO R. ABULOS, both of legal age, Filipino
Citizens and residents of Barangay San Antonio, Candon City, Ilocos Sur, after having been
sworn to in accordance with law, do hereby depose and say:

That we personally and intimately know one GENALYN A. NARTATES, being our close
friend and in the said locality;

That we know for the fact that she had been using surname NARTATES in all her
pertinent records and practically in all her legal transactions;

That upon verifying a copy of her Official Transcript of Record / Secondary Student’s
Permanent Record, issued by Candon National High School (CNHS), it was found out that her
surname and the surname of her parents was erroneously written as NARTATEZ instead of
NARTATES;

That we are executing this Joint Affidavit to attest to the truth of the foregoing
statements for the purpose of explaining the discrepancies made in the above-stated
documents and further establish that the names GENALYN A. NARTATEZ and GENALYN A.
NARTATES of the above-stated person is affiliated and refers to one and the same; and for all
legal intents it may serve.
