AFFIDAVIT OF LOSS

LTO DRIVER’s LICENSE OFFICIAL RECEIPT

I, ARISTON CAMARCE RAVELO, of legal age, Filipino citizen and

with residence address at No. 116 ME Aguirre Subd., Brgy. Malagasang
2-A, Imus City, Cavite, Philippines, after having been duly sworn to in
accordance with law hereby depose and state:

1. That, I am a duly-licensed driver in accordance with the

pertinent LAND TRANSPORTATION laws, rules and
regulations, and was issued a corresponding LTO
Professional Driver’s License with number D06-93-114152;

2. That, sometime on February 22, 2019, about 5:00 P.M., the

LTO Official Receipt of said driver’s license was inadvertently
thrown in a trash bin I thought it was no longer needed;

3. That, I exerted diligent efforts to locate my the receipt, but

the same proved futile and in vain, hence, it is forever
deemed lost;

4. Hence, I executed this affidavit of loss to herein attest to the

truthfulness of the foregoing facts and for the purpose of
securing a new LTO Professional Driver’s License Official
Receipt and to replace the lost one.

IN WITNESS WHEREOF, I hereby affix my signature this 9th day
