Being a student of the university and being enrolled in the subject, I agree and will follow these
guidelines:
1. The class arrangement will follow AP 4 (Full Face-to-Face) which states that 100% of the
classes will be conducted FACE-TO-FACE.
2. Following the arrangement in 1, MAJOR EXAMS, QUIZZES, and ACTIVITIES will be given
ON-SITE.
3. A student will be graded from 1.0 to 5.0 for MIDTERMS and 1.0, 3.0 to 5.0 for FINALS.
Note: NO STUDENT WILL RECEIVE A FINAL GRADE IN BETWEEN 1.0 - 3.0. In case a
student receives a grade of 1.0 for the finals, it denotes that the student has failed the subject.

4. The teacher can mark the student LATE if he/she arrives FIFTEEN (15) minutes after the
class has started. THREE (3) CONSECUTIVE LATE ARRIVALS in class will correspond
to ONE (1) DAY of absence.

5. The teacher can consider the student “DROPPED” if the ABSENCES will total to more than
20% of the total number of meeting hours or approximately EIGHT (8) meetings.

6. Inform instructors in case of health issues which may cause students to miss a quiz or major
examinations. Likewise, a HARD COPY of the MEDICAL CERTIFICATE (issued by
attending Physician at real time) must be submitted to the instructor upon reporting back
to school.
7. Only the death of IMMEDIATE FAMILY RELATIVES (parents and siblings) are considered
to be excused. Death of extended relatives (aunts, uncles, cousins, and grandparents),
however, will NOT be considered excused.

Should there be any concerns, I will raise them to my instructor beforehand for them to be addressed
when necessary.
