MEMORANDUM OF AGREEMENT

BETWEEN CAMELEON AND LGU

KNOW ALL MEN BY THESE PRESENTS:

This Memorandum of Agreement is entered into by and between:

CAMELEON ASSOCIATION, INC., a non- profit, non-stock organization

duly organized and existing under the laws of the Republic of the Philippines with
principal address at Brgy. Sablogon, Passi City, Iloilo, represented by its Executive
Director, HEIDE FOULC herein referred as the FIRST PARTY:

The LOCAL GOVERNMENT UNIT of____________________, herein represented

by Municipal mayor, ________________________ herein referred to as the
SECOND PARTY.

WITNESSETH

WHEREAS, the FIRST PARTY is a humanitarian organization providing

protection and comprehensive rehabilitation services to young girls in Western
Visayas who have suffered sexual abuse and maltreatment. The organization strives to
promote empowerment and development of children and their families through
education, psychosocial interventions an advocacy; (coverage: Western Visayas)

WHEREAS, the FIRST PARTY undertakes a Residential (In-house)

Rehabilitation Program that aim to rehabilitate sexually abused and maltreated girls to
ensure a successful reintegration to their families and communities;

WHEREAS, the FIRST PARTY works with local government units, national
government agencies, individual sponsors, corporate and institutional donors,
concerned individuals/ groups, civil society organizations, biological/ foster families
and communities to facilitate healing and recovery of abused and maltreated girls;

WHEREAS, the SECOND PARTY is mandated to carry out policies,

programs, projects and activities through a committed public service to attain
progressive and sustainable development for its people particularly, the protection and
wellbeing of children and youth;
 WHEREAS, the SECOND PARTY collaborates with national government
agencies, private and public sectors and civil society organizations whose purposes are
congruent with its vision, mission, goals and objectives;

NOW THEREFORE, for and in consideration of the foregoing premises, the

parties hereby agree on a partnership to rehabilitate sexually abused and maltreated
girls and have accepted this challenge to be of service to these girls in difficult
circumstances with the following terms and conditions to wit:

THE FIRST PARTY SHALL:

1. Provide a safe, calm, secure and peaceful environment allowing the girls to
confront their dark experiences in an atmosphere of healing and self-
acceptance.
2. Provide basic needs of the girls such as food, clothing, shelter education,
medical/ dental, personal hygiene, care, attention, among others.
3. Engage the girls in various therapeutic learning activities such as psychosocial
interventions, sports and recreations, moral and spiritual nourishment, among
others, that awaken in them self- confidence, independence and discipline.
4. Provide legal support and assistance to facilitate healing and recovery of the
girls.
5. Provide an interdisciplinary team in the center composed of Housemothers,
Social Workers, Cook and Security Staff to maintain a 24 hours monitoring
scheme to ensure the safety and well-being of the girls.
6. Provide to the local social worker of the list of requirements for admission in
Cameleon.

THE SECOND PARTY:

1. Provision of twenty thousand pesos (20, 000.00) counterpart of LGU

annually per client admitted by the FIRST PARTY. Payment shall be made
within 30 days upon receipt of the billing from the FIRST PARTY. (mode
of payment)
2. Submit all necessary documents upon admission such as NSO birth
certificate, educational documents, psychological test evaluation, medico-
legal certificate, complete medical result/records, pertinent legal documents,
endorsement letter of mayor, transfer summary report and referring letter of
agency (if applicable), and social case study report.
3. Proactively monitor the progress of the child.
4. Participate and cooperate with conferences conducted by the FIRST
PARTY relative to the resolution of issues/ concerns about the child and/ or
about her general well-being.
 5. Facilitate the filing of legal cases, follow up of court hearing and
proactively support and assist the child throughout the legal process in
coordination with the FIRST PARTY.
6. Facilitate the reintegration of the girl/s in coordination with the FIRST
PARTY and identify possible custodian/s/ foster families.
7. Take the lead role in preparing the families of the girls for reintegration.
8. Make formal endorsement about the visit of the family of the child to the
FIRST PARTY prior to the actual visit.
9. Adhere to the Child Protection Policy of the FIRST PARTY.

IN WITNESS THEREOF, the parties have here unto signed this Memorandum of
Agreement on the ______ day of _________ 202__ at________________, Philippines.
