SUBJECT : NOTICE OF DECISION

This is with reference to an incident of DERELICTION OF DUTY reported against you by way of
SLEEPING DURING WORKING HOURS AND FAILURE TO WORK ON AUTHORIZED
OVERTIME WITHOUT VALID REASONS in GROSS VIOLATION of the COMPANY’S CODE OF
DISCIPLINE.

You stated in your written explanation received by this office last November 30, 2023, you operated
Dumptruck #282 to Lapaz Dike on the evening of September 16, 2023, from 6:00 to 8:00 PM, for the
purpose of dumping in the designated area. When it began to rain around 8:00 PM, you parked
Dumptruck #282, and the Backhoe unit was also parked. You patiently waited until 10:00 in the evening
for the backhoe to resume its operations; however, it had not yet resumed its activities. Thus, there is no
evidence available that confirms the truth of the allegation.

In view of the foregoing, you were not at fault in the incident. Hence, this is to formally inform that you
are ACQUITTED in this case.
