Scope of Services

SCOPE OF SERVICES

1. BACKGROUND & JUSTIFICATION

Mutanda Mining has been in care & maintenance for the last 2 years and has since commenced with the restart of the
Phase 2 circuit. In line with the future production plans for Mutanda Mining, we will require additional streams within
the plant to be operational.

The Supply Chain Management (SCM) constructed 6528 sqm of Concrete slab & Base plinth Foundation for the new
Sulphur acid & reagent Lime storage due to the increase in the reagent demand to improve their internal product
management. The Structure & steel works of the new storage warehouse will be erected on the existing Concrete slab
built at the lime area next to the Mining and Project office.

2. SCOPE OF WORK SUMMARY

These works under this Contract include, but are not limited to, the following:

 Site Setup & Mobilisation


 Structural Steelwork Construction & Concrete around the building
 Supply materials (requested in the BOQ)
 All anchors’ bolt installation, including alignment.
 Housekeeping.

3. INTRODUCTION TO SERVICE ITEMS

3.1 Contractor Supply


 The Contractor must supply all necessary Constructional Plant, materials management and supervision,
labor, and services to complete the Services, which includes but is not necessarily limited to the Items
described herein.
 Mobilisation and demobilisation
 Provision of risk register mitigation measures and ongoing updates
 Plant, Equipment, and working tools required to perform the work on schedule
 Fully qualified and experienced Personnel
 High-quality work performed as per MUMI standards is required
 Personal Protective Equipment (PPE) for its Personnel and the equipment (extinguishers, lights, safety
harnesses, etc.) in accordance with the Company’s standard
 Fuel, lubricant, spare parts, consumables, and maintenance equipment.
 Food and water to the employees on-site, clean-up after consumption of food, and proper disposal of
trash.
 Medical care and insurance for the Personnel and the equipment
 Facemasks and hand washing stations for workers
 Transport materials from storage areas to the site.
 Provide a risk assessment for all planned activities required to complete the Services Safety

 Work permits and visas


 Participate in daily safety talks and discussions when on site
 Communication means for Contractor’s Personnel
 Copies and records of induction, training, competency, medical, etc., upon request by MUMI.
 Accommodation, meals, and laundry
 Medivac for its ex-pat personnel
 Storage of materials
 Security of site, materials, offices, employees
 Transport employees in their own transportation safely to MUMI Site and work areas.
 Work during nights with adequate lighting.
 Manage employees’ fatigue during extended shifts and double shifts.

3.2 Service description methodology and measurement of work


 Provide Construction Methodology and baseline schedule for approval within one week of the fully signed
contract.
 Provide risk assessment for approval within one week of the fully signed contract.
 Provide daily productivity report based on Bill of Quantities.
 Provide daily progress reports, weekly schedule updates, and labor histograms.
 Provide Rigging study and plan.

3.3 Site Operations


 Follow MUMI rules and guidelines.
 Effective communication with MUMI in all matters.
 Beware of mining equipment and do not interfere with mining operations.
 Blasting in the open pit.

4. MEASUREMENT, PRICE, INVOICING, AND PAYMENT

 Costing and measurements as per BOQ.


 Certificate payment is to be made as per the agreed BOQ template.
 Invoicing every month based on MUMI-approved measurement methods.
 Scope variations and Site Instructions to be discussed, verified, and approved by MUMI.
 Re-measurement of quantities to be performed at the end of services.
 Invoices will be paid within thirty (30) Days following receipt by the Company's Finance Department of
each invoice of the Contractor.
 The contractor has to revise/reduce the price related to the market that is to be verified and approved by
the project team and Contract representative

4.1 General
Scope of Services

The rates and prices contained in the Contract Price Breakdown will be deemed to be fully inclusive prices for
the prescribed Services to meet design, overheads, drawings, specifications, and requirements, inclusive of all
provisional items:
4.2 Invoicing

 Draft invoice format, supporting documents (like completion certificates, etc.), and required
date of receipt
 Invoice to be approved in 15 days,
 Invoice to be paid in 30 days from date of approval,

6. DETAILED SCOPE OF WORK

The SCM department requested erecting the Steel Works and Electrical on the existing Concrete Slab (sqm
6528 ) at the Lime area next to the Mining Project. The project office approached the SCM to do the
warehouse's design development and construction management. This document will establish the
engineering and construction scope of work and general standard project development practices.

The site Location from the map is Shown in the picture below. The site coordinates are -10° 46’37” S, and 25°
48’46” E.

Scope of Services

The Project includes Installation activities:


1) The contractor shall provide equipment, fully qualified and experienced personnel, and high-quality
work per Company Standards to install all the Electrical equipment – plumbing & civil works, and
camps refurbishment as requested by the project owner.
In this contract, the contractor will supply all materials related to the BOQ.

The Scope of the services to be performed shall include, but not be limited, the following:

I. Steel works: The contractor shall provide equipment, fully qualified and experienced personnel, and
high-quality work per Company Standards to construct the new reagent storage facility on the
Existing lime area, as requested by the project owner.
a) Supply and install the main structure & the secondary structure steel (UB Column 254x146x37kg/m
of 10.140 m - UB Column 254x146x37kg/m of 6.616 m - UB Column 254x146x37kg/m of 4.500 m)
b) Supply and install the Roof Frame Beam long of 40 m in the Middle and 24 m on the extreme
(406*140*39kg/m UB & 305*102*25kg/m UB)
c) Supply and install Knee bracing of 50*50*L –Lip Channel of 750*75*20*2 mm
d) Supply and install the wall and Roof Bracing of 50*50*5
e) Supply and Install the Roof sheeting IBR Galva 0.40 mm. Ensure to use 10 o slope
f) Supply and install all the following accessories where required as per BOQ and Drawing

 Connecting Plate 20 THICK- M20 BOLTS Grades 8.8 - 24 DIA Holes

g) Remove and clean all wastage and dispose
II. Concrete Walkway:

a) Prepare and build concrete at 30 MPa upon a slab of 5000 mm wide and 150 mm thick with mesh reference
395 shall be installed around the building to keep water away (the contactor shall install a DPM -375 and use a
green sheet or similar on topsoil poisoned mattress before casting concrete
b) Construction of stormwater concrete drains that will be covered by heavy-duty gratings

1. Miscellaneous:
a. The contractor shall ensure that his personnel are trained, competent, and hold the relevant
certifications
b. The contractor shall coordinate with the engineer on the service delivery schedules
c. The contractor shall ensure that the services are performed and completed in accordance with the
agreed service delivery schedule
d. The contractor’s working hours shall be that of MUMI’s working hours unless discussed and agreed
otherwise between MUMI and the contractor

e. Daily housekeeping and proper barricading are compulsory to secure the site
f. All rubble shall be disposed of as per MUMI environmental policies
g. The contractor shall abide by MUMI’s requirements with regard to safety, permits, inductions, SOP,
etc.
h. A detailed risk assessment (RA) report and mitigation measures are to be provided to MUMI within 7
days of signing a contract. This RA will be maintained, and appropriate actions must be implemented
throughout the project’s life and reviewed weekly at the construction progress meetings. Updates
must be provided daily at 8 am without fail.
i. All contractor personnel presence on site or attendance in the progress meetings shall be recorded
on daily diaries, verified, and signed off by the Engineer or his representative daily.
j. The contractor shall follow the Covid-19 safety protocol as per MUMI standards.
k. The contractor is to erect site signage in accordance with the code of practice that will clearly display
the project and the contractor’s information. MUMI to provide details of the signage board

6.1 SCOPE OF WORK FUNDAMENTALS


 Detail the Scope of Work requirements
 Follow MUMI-defined targets per the detailed scope of work and bill of quantities.
 Reference applicable Drawings and Specifications
 Drawing design given and subject to clarification with the contractor for final detailed Design
 Specifications to be provided during the site kick-off meeting after the contract has been awarded.
 Confirm Mobilisation and establishment requirements
 MUMI to provide an area for site establishment. Contractor to provide own source of electricity and
water. Contractor to de-establish after completion of contract services.
 All Contractor employees to undergo MUMI inductions.
 All Contractor operators must undergo MUMI assessments and get authorization to conduct work on
MUMI premises.
 Provision of management and supervisory personnel
 Management qualification, experience, supervisor skill sets, personnel trade skills, and tickets. CVs of
key personnel to be included in the submission.
 Explain Site operations
 Work location is Lime area Next to the Mining and Project office.
 Access Roads available.
 What Quality management, Quality assurance, and testing is required
 All Quality documents, including test results, are to be updated daily and included in the Data Packs
Contractors.
 Subcontractors

 Contractor to provide details of sub-contractors as requested by the MUMI representative


 Construction plant and equipment requirements
 Contractor responsible for mobilizing plant and equipment to site, including availability, condition
reports, and means of establishment.
Scope of Services

 All equipment photos are to be submitted during the tender.


 All plant and equipment to carry valid certificates, service history, licenses, etc., and other documents
requested by MUMI.
 MUMI to inspect and approve vehicles before work commencing.
 Standards (applicable to site)

 Contractor to follow MUMI Site compliance standards for all related matters
 Include Program of Services with Key Dates
 All works to be completed before July 2023.

 Provide Key performance indicators (KPIs) to measure the Contractor/Supplier


 Daily productivity as per Bill of Quantities, taking into consideration Safety, Cost, Work Performance,
Reporting, and Site standards compliance
 Reporting and Meeting requirements
o Kick-off meeting after Contract has been awarded or fully signed.
o Daily on-site with Contractor site supervisor (work/operations related)
o Weekly with Contractor site Manager (review Contractor Performance)
o For recurring services over a longer-term contract:
o Monthly with Contractor site Manager (works approval and Draft invoice review)
o Quarterly with both Senior Management Teams to review Contract obligations.
 Applicable codes, rules, and regulations
It is the contractor’s responsibility to deliver the Construction Work defined in these Specifications in
accordance with the applicable requirements of international and national laws, codes, rules, regulations,
ordinances, and standards, including, but not limited to, those described below:

o BS-British Standards
o EN- European Standards
o IBC-International Building Codes
o ISO-International Organization for Standardization
o ASTM (International)
o American Society for Testing Materials
o SANS-South Africa African Standard

Other recognized international standards are accepted if the requirements specified in this scope of work are
met. The Contractor must clearly indicate in the technical documentation which codes, Standard and Services,
in compliance with the builder’s Quality Assurance Certificate, will be presented for the goods or materials to
be used on the project.
