1.0 PURPOSE
To define reporting work-related accidents or incidents occurring in the organization.
2.0 SCOPE
It applies to all types of near misses, incidents, and accidents in the organization.

5.0 PROCESS
5.1 Notifying the Near Miss / Incident / Accident
All employees must inform the emergency team members about near misses/incidents/accidents at the earliest to
the emergency team members over the phone or through other means.
5.1.1. Near Miss / Incident / Accident reported within premises
 Proceed to the identified reported near miss/incident/accident location.
 Evaluate the nature of the hazard or the potential cause of the reported hazard / near miss.
 If the corrective action / preventive action needs an extended action time, isolate the identified hazard with a
tag/notice.
 Follow up necessary action to be completed at the earliest.
 Record the hazard / near miss in the risk, near miss, and incident investigation report.
 Record the details in the incident database.
5.1.2 Near Miss / Incident / Accident reported outside premises
 Inform the First aid personnel about the incident/accident.
 Rush to the incident/accident location.
 If the injury is severe, call the ambulance for medical assistance.
 Secure the scene of the incident if it is related to fire, severe physical injury, etc.
 Ensure that the associated material, equipment, etc., are not disturbed in the incident location.
 Provide first aid, if required.
If the injury is severe, provide first aid and try to comfort the injured staff and stay with him until the ambulance
arrives and take charge of the injures team.

5.2 Incident / Accident Takes Place Outside (Vehicle Accidents)


 Inform the Ambulance and Police about the incident/accident.

Recording the incident/accident


 Record the incident/accident for an investigation (Take photographs in all angles)
5.3 Investigating the Incident / Accident
 Appoint the investigation team.
 Inform the IMS Representative about forming an incident investigation team.
 Form the incident/accident team and communicate to the team.
While forming the team, ensure the following:
 At least one team member should have sufficient expertise on the subject matter related to the incident. If
an incident/accident happens in the server room, a person should know enough about the electrical
hazard and associated risks.
 The management representative will be a member of the team.
 Set a deadline for finishing the investigation and submit the report to the top management.
Important: If an incident/accident happens while traveling for business, a police investigation report will be
considered a final report.
5.3.1 Collecting Investigation Evidence.
 Visit the incident/accident scene.
 Collect the evidence through the following primary sources.
o People present at the time of the incident/accident location.
o Position of the equipment/material/person involved.
o Look at the damage that occurred.
 Take photographs of the incident/accident scenes.
5.3.2 Evaluating evidence.
 Prepare incident/accident reports using the forms attached to the documented information.
 Attach all evidence with the report.
 Update the top management for proposed actions.
5.4 Implementing Corrective / Preventive Actions
Ensure implementation of appropriate corrective and preventive action based on:
 Root cause of the incident/accident.
 After the investigation, check for proper action.
 After the corrective action, calculate the loss time injury.
 After that, submit the report to the top management and the proposed preventive action.
