Your attention is being called in reference to the Incident Report received from
Property Management Department dated April 3, 2024 regarding the allegation(s) stated as follows:

Reportedly, you didn't handle the situation appropriately when instructing an employee
to close the door on the second floor, despite being the second person to exit through it.

Corrective Counseling was made between you and your Manager/Supervisor to discuss the deviation you
committed against the company’s Code of Conduct and Discipline to wit:

Employee Acknowledgement

In the course of the Corrective Counseling, I acknowledged the deviation I committed and I understood that
my action caused inconvenience to my department. Moving forward, I shall adhere to the guidelines set by my
department and shall ensure to avoid repetition of such incident.

In view of the above, both parties agreed of the imposition of Verbal Warning as disciplinary action for the
deviation committed and that repetition and/or committing the similar or related deviation shall be dealt with
accordingly based on our Code of Conduct and Discipline.
