We, a Grade 12-EIM/HE students enrolled in the subject Practical Research 2 and is
conducting a research entitled, “AN ANALYSIS ON THE STATUS OF VITO NATIONAL
HIGH SCHOOL DROP-OUTS,” it aims to (a) determine the demographic profile of the
respondents; (b) determine the current status of the VNHS drop-out from S/Y 2019-2022,
2020-2021, 2021-2022; and (c) know the experiences of the respondents in coping as a
high school drop-out.

Regarding this matter, we would like to ask your approval to please allow us to conduct a
survey among the drop-out students of various school years. Attached is the sample
questionnaire for your perusal and rest assured that all of the information will be treated
with confidentiality.
