THIS IS TO CERTIFY THAT, “Dogger”, a one-year old, male, brown

Aspin owned and raised by Christelle Mae A. Cuevas, MD, MBA, has been
administered with the Anti-Rabies Vaccine by Antonieta Cosmiano, MAO
Farm Supervirsor, on August 18, 2020 at Zone 4, Lanuza, Surigao del
Sur.

THIS IS TO CERTIFY FURTHER THAT, based on the records

available in the office, this is the second occasion that “Dogger” has been
administered with the Anti-Rabies Vaccine with his first vaccination on
August 16, 2019.

This certification has been issued for the purpose of the transport of
the canine from Lanuza, Surigao del Sur, to Pasig City, Metro Manila, and
for any other legal purpose it may serve.

Issued this 24th day of August 2020 at the Municipal Agriculture’s

Office, Zone 2, Lanuza, Surigao del Sur.
