The support offered by the Disability & Access Support Unit is tailored to meet your
individual needs. You may require support/assistance in one or more of the following
areas:

 tests, exams or practicals

 special examination provisions for the HSC
 study planning or course content support
 more general support
 assistive technology

To register for Disability & Access Support, I require documentation regarding the nature
of your disability or medical condition. The documentation can be provided by your
General Practitioner (GP), Psychiatrist or Psychologist. The documentation assists me to
identify the appropriate support for your specific study needs. All information is kept
private and confidential.

I also require you to complete the attached Student Registration Form. The information
you provide will assist me to identify the most appropriate support to help you manage
and succeed in your studies. All information is kept private and confidential.

I have a set of textbooks available for loan. Please contact me if you are unable to
source the textbooks.
