TO WHOM IT MAY CONCERN:

The FLA-Student Council is ta non-sectarian and non-political organization. As the highest Student
Organization among other organizations in the Institution, it shall govern the relationships between and among the
organizations of the school with respect to the interests of the students of Fiat Lux Academe and shall conduct
activities that will boost their intellectual abilities, talents, and skills.

In pursuance to its utmost duty, the FLA- Student Council will be having the annual FIATHLETICS 2024 on
February 13 to 15, 2024, for all Senior High School students with the theme, “Prevailing Inner Depths of Trials in
Flaming Excellence”. This event will have numerous activities and competitions, that will help establish unyielding
relationship among Luxians,.

In this light, we will be having try-outs on February 5, 6, and 7, 2024, at 3:00 PM to 6:00 PM at the following
venue: Quadrangle for selected sports in relation with the said event. Our deepest gratitude is for your child to be part
of this notable event.

PARENT'S WAIVER

This is to inform you that I have approved the participation of my child/ward in the FIATHLETICS 2024 try-outs to be
held on February 5, 6 and 7, 2024 in Fiat Lux Academe Dasmarinas Campus at 3:00P.M-6:00P.M. Having considered the
benefits that my child/ward will derive from his/her participation in the said activity and with the understanding that every
precaution will be taken by the organizers and the teachers concerned to ensure his/her safety during this activity. I will not hold
any of the organizers and the teachers concerned responsible for any untoward incident that may happen beyond their control.
