GENERAL DIRECTIONS: This survey questionnaire is created by the Grade 12 ICT – 2 for the quantitative
study entitled “Web-Based Health Tracker Record Management System” the main objective of this research is
to develop a web-based health tracker record management system. This system aims to speed up the process of
recording and managing health data for improved accessibility and efficiency.

Participation in this survey is voluntary. Respondents are allowed to withdraw if they feel to do so.

In accordance with the Data Privacy Act of 2012, all the responses that will be gathered will not be linked to
any information about the participants and will be strictly treated with the utmost confidentiality. The data
collected is for research purposes only.
