I, the undersigned, will not be able to claim my Professional Identification

Card [PRC License] due to an avoidable circumstance. Please accept this letter as
confirmation that I authorize my colleague Ms. Vera Alva C. Villalas, who is a
Professional License holder herself, to claim my PIC in my behalf.

I have no objection for her signing documents if there are any.

I am willing to enclose any copy of my identification to prove that this request

is valid. Let it be known that I have given her a xerox/photo copy with my affixed
signature below.

For security purposes, she hereby presents and/or provides a copy of her
Professional ID Card, if deemed necessary.
