http://www.ustraveldocs.com/ to create a new deposit slip. In the 'Reference No.' field, it is
mandatory to put in the personal CGI reference number of the applicant, if not the
payment cannot be allocated to the applicant.

Cash Deposit Instructions -

To pay your nonimmigrant visa (NIV) application fee in cash at any DFCC Bank, this deposit
slip must be used before the expiration date, as noted above.
Print one copy of this deposit slip. DO NOT PRINT MULTIPLE COPIES OF THE DEPOSIT SLIP.
Take the printed deposit slip with you to pay your fee. Be sure that you pay the exact
amount indicated on the deposit slip.
In the DFCC Branch, please complete the DFCC Bank deposit slip, including the CGI
Reference in the 'Reference No.' field on the deposit slip. DO NOT OMIT THE CGI
REFERNCE, otherwise the payment cannot be allocated to you.
If you are applying for multiple visas for your family or group, only ONE payment is
required.

Instructions for DFCC Bank Branches:

Your visa fee payment receipt in non-transferable and therefore cannot be used by anyone
else.
After receiving your payment, the cashier will give you a receipt. Save your receipt as proof
of payment.
The payment shall be valid for 12 months from the date of payment.
