AFFIDAVIT OF UNDERTAKINGS
We, _ Rufino F Melo III and __ Ma. Liza G Melo_, a Filipino, of legal age, with
property address located at Blk 1, Lot 13, Phase 1, Southpeak, Brgy. San Antonio, San
Pedro City, Laguna, After having been subscribed to an oath, hereby depose and say;
1. That, We are the property owner of the above mentioned property;
2. That, We were not able to process the transfer of the Property Tax under our
name;
3. That the documents listed here under are the only available documents, We are
able to secure from the Office of the Registry of Deeds of San Pedro City, to wit;

a. Deed of Sale;
b. Real Property Tax Receipt; and
c. Certified True Copy of the Title with TCT No.210075

4. That, such documents will be used as references or attachments in connection

with my request for a copy of Certificate Authorizing Registration (CAR) which
will be issued by the Office of the City Assessor of San Pedro City for my
personal use and or documentation.

5. That, We hold ourselves criminally liable for perjury and dishonesty should any
statements made herein are inaccurate or later to be found violated any
provisions of the Law.
