name;

able to secure from the Office of the Registry of Deeds of San Pedro City, to wit;

a. Deed of Sale;
b. Real Property Tax Receipt; and
c. Certified True Copy of the Title with TCT No_________

4. That, such documents will be used as references or attachments in connection

with my request for a copy of Certificate Authorizing Registration (CAR) which
will be issued by the Office of the City Assessor of ____________________ for
my personal use and or documentation.

5. That, We hold ourselves criminally liable for perjury and dishonesty should any
statements made herein are inaccurate or later to be found violated any
provisions of the Law.
