Installation Closeout Documentation Checklist

❑ Height Confirmation
›› Antenna tip, bottom and RAD
›› Bottom of mount where attached to tower
›› If work is at top of tower, additional heights required:
›› Height to top of steel
›› Height to top of highest appurtenance
›› Height to bottom and top of lighting beacon (if present)

❑ As-Built / Redline Drawings (should show changes or state “installed as depicted”)

❑ Building Permit Signoff or Certificate of Occupancy (if not required, a signed affidavit is acceptable)

❑ Photos – Ground Work (please provide photos applicable to the work being performed)

›› American Tower sign on compound gate

›› Open trench, showing entire length with conduit
›› Backfilled trench, showing fabric and stone replaced
›› Pre- and post-photos of ground work
›› Ground ring
›› Pre- and post-photos of any damage that was repaired (access road, fence, etc.)

❑ Photos – Tower Work (please provide photos applicable to the work being performed)
›› American Tower sign on compound gate
›› Antenna equipment labels, showing model numbers (panels, RRUs, TTA/TMA, omni, etc.)
›› Views from tower looking to back of each sector array, showing antenna/RRU/TTA/TMA placement
›› Views from ground, showing entire tower
›› Views from ground, showing close-up of each sector
›› If install is near any lighting equipment, views showing lighting equipment is not obstructed (beacons, markers,
junction boxes, etc.)
›› Views showing safety climb is tight and placed back in cable grabs and is not rubbing or engaging customer’s
equipment

Concealed Towers
❑ Photos – Concealed Towers (please provide photos applicable to the work being performed)
›› Pre- and post-photos of any concealed parts removed/replaced from the tower, ensuring all bolts are properly installed
upon completion
›› Pre- and post-photos of customer equipment being installed/replaced
›› Views showing tape drop of customer equipment height(s)
›› Views showing location of main feedline termination and transition to jumper
