Greetings!

The City Government of Tagum through the office of the undersigned will commence its
Septic Desludging Program by the 2nd Quarter of this year. This effort is our
compliance to the Code on Sanitation of the Philippines (PD 856) and Philippine Clean
Water Act (RA 9275) in order to protect and improve the water quality and sanitation of
our water bodies for the general welfare.

In view of this, we would like to respectfully request a List of Requirements or any

Proforma for the “Application for Septage Stationary Service Providers and for
Mobile Service Providers” for us to acquire an Environmental Sanitation Clearance
(ESC) from your department.
