RELEASE AND WAIVER OF LIABILITY AGREEMENT

Gianelle dela Cruz, acknowledge that I have voluntarily applied to participate in the following
activities.

I understand that INGCO Service Center will not be responsible for any liability or damage upon
my request to pull out the unit with

RF# : SC15869 / SC16152

Unit Model: AG750282 / AG8006-2
Serial # : FADED / 17092610530
without repair and this will be Labeled as RETURN WITHOUT REPAIR in the Ingco Service Center
System.

I agree that INGCO Service Center will release the unit without replacing all of the needed parts
so we will not require any functionality/quality testing from INGCO Service Center.

I am fully aware of the risks and shall take full responsibility for any issue that may arise using the
unit that did not go through the proper functionality/quality testing by the INGCO Service
Center.

I agree to pull out the UNIT within 1 week or the unit will be subject to disposal.

I HAVE CAREFULLY READ THIS AGREEMENT AND FULLY UNDERSTAND ITS CONTENTS. I AM
AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT BETWEEN MYSELF AND INGCO
SERVICE AND I SIGNED IT OF MY OWN FREE WILL.
