DECLERATION LETTER

To whom it may concern,

This letter is to verify that Ms Anum Urooj, ID No. 00176791A has worked for our company since 05th
October 2020 on Full Time Basis. She is presently a Health Carer Assistant in the community.

Due to recent restrictions Ms Anum Urooj has no option but to take care of her children. The nature
of her job cannot opt for teleworking and we do not receive any covid wadge supplement on her
behalf.

During this period, she will be taking unpaid leave as per our agreement.

Kindly consider this application for the COVID-19 parent benefit.
